School to Adult Transition - Current Initiatives Back to High School to Adult Transition Homepage Louisiana Statewide Transition Project (LSTP): The LSTP is a five-year federally-funded systems change project dedicated to building and sustaining statewide transition services. The goal of the project is to create a multi-level framework of resources and activities for lasting transition services through collaborative activities involving youths with disabilities and their families. (8/1/96 7/31/01) Louisiana: Healthy and Ready to Work (LHRW): The LHRW is a five-year federally-funded project with a goal to build the self-determination skills of adolescents and to maximize the coordination of state and local educational, health and medical, and rehabilitation services. (10/1/96 9/30/01) Louisianas Self-Determination Research Project (LSDR): The LSDR is a three-year federally-funded project with a goal to use a longitudinal, multiple-phase, group research design to investigate both the short-term and the long-term effects of self-determination instruction and/or participation in a Youth Leadership Forum (YLF) on self-determination skills and behaviors, IEP (Individualized Education Program) participation and involvement, and transition outcomes of adolescents with disabilities.
